Vitiligo

What is an effective treatment when vitiligo is spreading rapidly?

During the active phase when vitiligo is spreading rapidly, treatment with oral steroids can be considered to first suppress disease progression. After the lesions stabilize, color recovery can be induced by combining excimer laser or UV therapy.

How Do Oral Steroids Help in Vitiligo?

The most important effect is to suppress the progression of the disease so that vitiligo does not spread further.

In rapidly progressing vitiligo, it is necessary to first prevent new lesions from appearing or existing lesions from enlarging by modulating the immune response.

In oral steroid treatment, disease progression has been reported to be suppressed in approximately 80–90% of patients.

After the disease has stabilized, ultraviolet light therapy, excimer laser, or tacrolimus ointment can be combined to induce repigmentation.

In other words, oral steroids can be seen as a treatment that stabilizes active vitiligo so that subsequent repigmentation therapy can be effective, rather than a treatment that directly fills in pigment.

How Are Oral Steroids Taken?

The method and duration of administration are adjusted according to the rate of vitiligo progression and treatment response.

In cases with clear active phase, after an initial period of intensive treatment, oral mini-pulse therapy—taking the medication only on certain days of the week—may be used once the disease stabilizes.

If new white patches continue to appear or existing lesions enlarge during treatment, the dosage or administration method can be readjusted.

The treatment duration can also vary, from as short as a few weeks to several months, depending on the degree of progression.

Therefore, what is important is not applying the same fixed regimen to all patients, but adjusting the treatment while actually checking whether the spread of vitiligo has stopped.

What Are the Side Effects of Oral Steroids?

The longer the dosage and treatment duration, the higher the potential for side effects, so regular monitoring is necessary.

Relatively common symptoms that may occur include the following:

- Weight gain and changes in appetite

- Poor sleep quality

- Temporary mood changes

- Heartburn or gastrointestinal discomfort

With long-term use, the risk of osteoporosis may increase due to decreased bone density, and very rarely, side effects such as avascular necrosis may also occur.

Therefore, if uncomfortable symptoms occur during treatment, you should discuss them with your Dermatologist to adjust the dosage and treatment plan, and it is important that the patient does not change the dosage or stop the medication on their own.

Extra Precautions for Diabetes, Children, and Women

The aspects to check when using oral steroids differ depending on underlying conditions, age, and sex.

① Patients with diabetes or osteoporosis

Steroids can raise blood sugar or affect bone density, so they must be used more cautiously. If necessary, other systemic immunomodulatory treatments may be considered.

② Pediatric patients

Because there may be concerns about growth, it is important to regularly monitor growth status during the treatment period. Methods that reduce the number of doses, such as pulse therapy, differ from long-term daily administration.

③ Female patients

In some cases, the menstrual cycle or pattern may temporarily change during the treatment period. In most cases, this returns to normal after treatment ends, but if the change persists, it is advisable to discuss it with your attending Dermatologist.

What Matters Most in Oral Steroid Treatment?

It is to accurately determine the active phase in which vitiligo is actually spreading and to use the medication appropriately for the necessary period.

Oral steroids are not a treatment used for all vitiligo patients.

It is necessary to first check whether new lesions are continuously appearing, whether existing lesions are enlarging, and whether the patient has any underlying conditions that make steroid use difficult.

Also, after the disease stabilizes, rather than continuing oral steroids, it is important to shift the treatment goal to the repigmentation stage using excimer laser, ultraviolet light therapy, or topical treatments.

Ultimately, the key to oral steroid treatment is not to use it strongly for a long time, but to use it appropriately during the period when vitiligo is spreading to stop its progression.
