Jowl lines are not caused by a single factor but by the cumulative aging of bone, muscle, fat, ligaments, and skin. As we age, the jawbone (mandible) gradually resorbs, reducing the foundation that supports the tissues. The fat layer in the cheeks shifts downward and redistributes, and sagging tissue gathers around the points where the ligaments hold the skin, causing the area beside the mouth corners to appear puffy and droopy. When the action of the muscle that pulls the mouth corners downward (depressor anguli oris, DAO) and reduced skin elasticity are added, a sullen expression becomes fixed.
Due to facial structural characteristics, East Asians tend to have relatively more noticeable sagging around the mouth area. In other words, jowl sagging is not simply caused by "weight gain" but is a result of structural aging, so diagnosing which layer's problem is dominant is the starting point for improvement.
