Can excimer laser treatment for vitiligo reactivate cold sores (herpes simplex)?
Reactivation of Herpes Simplex Virus Secondary to Excimer Laser Treatment in a Patient with Vitiligo
In short
This is a case where dormant herpes simplex virus was reactivated after excimer laser treatment for vitiligo. It is a rare but noteworthy side effect.
Sometimes, when treatment seems to be going well, an unexpected event occurs. There was a patient receiving excimer laser treatment for vitiligo. A herpes simplex (herpes) blister reappeared near the treatment area. This case reminded us that this treatment, which uses ultraviolet light, can awaken a dormant virus. We have documented this case so that others may refer to it.
Q1 Why can laser therapy reactivate herpes simplex?
Excimer laser is a treatment that uses ultraviolet light. However, ultraviolet stimulation can be a trigger that reactivates a dormant herpes simplex virus. Once infected, the herpes simplex virus hides quietly in the nerves. Then, when triggered by stimuli such as sunlight, ultraviolet light, stress, or fatigue, it reappears as blisters. If you are someone who often gets cracked lips or frequent blisters around the mouth, the same thing can happen at the laser treatment site. In this patient, the vitiligo treatment itself was not a problem, but the herpes recurred in that area. Therefore, for patients with frequent herpes outbreaks, I inform them of this possibility before treatment. If necessary, we also prepare with antiviral medication. Although it is not common, knowing about it allows you to respond quickly without panic.
Closing remarks
Even with an effective treatment, unexpected reactions can occur depending on the person and situation. Accumulating such experiences allows us to assess risks before treatment. For patients with frequent herpes outbreaks, we can also prepare preventive measures. This small case serves as a guide to prevent discomfort for other patients.
