Thermage® FLX uses radiofrequency (RF) energy to broadly heat the dermal layer, while Ultherapy PRIME® uses high-intensity focused ultrasound (HIFU) to reach the SMAS layer at about 4.5 mm depth—this is the biggest difference. The SMAS layer is the fascial layer addressed in facelift surgery; Ultherapy PRIME® creates coagulation points in this deep layer to lift sagging tissue. In contrast, Thermage® FLX contracts collagen throughout the dermis and stimulates regeneration, giving it an advantage in improving skin surface elasticity and texture.
| Category | Thermage® FLX | Ultherapy PRIME® |
|---|---|---|
| Energy | Radiofrequency (RF) | Focused ultrasound (HIFU) |
| Main target layer | Dermal layer | SMAS layer (approx. 4.5 mm) and dermis |
| Primary purpose | Tightening, elasticity, fine lines | Lifting, sagging improvement |
| Onset of effect | Gradual over 2–6 months | Gradual over 2–3 months |
In other words, Thermage® FLX is closer to "tightening the skin," while Ultherapy PRIME® is closer to "lifting sagging skin." The two procedures are not about which is better but rather complementary, targeting different depths and layers.
